Microsoft Access (no MacOS version) or

If you want to create a form like MS Access, you may have fewer choices.

Virtualization software such as VMWare and Windows+MS Access may be the most convenient.
(I can eat the CPU, but I think I can operate it seamlessly on my Mac.)
